2. Fill out the Entry Form and enter THAT DAY'S DR. OZ TOPIC. Your must enter each box to qualify! That's it!

One Entry Per Day (24 hour period)!!

3. A winner will chosen by random computer drawing of completed entries on Tuesday, November 24, 2009 by 5pm.

4. Contest Winner will be announced Wednesday, November 25th during THE DR. OZ SHOW (between3-4PM) - and - right here on www.KEZI.com. We will also contact winner by email or phone.
